Once this form has been completed & submitted and we have approved it, we will invoice the person named above for their share of the course. Once their contribution is settled we will book you on to the course. Please note, courses get filled very quickly so please ensure we receive payment promptly. If a course is filled we will have to look at alternatives.

I will commit to working a minimum 1 year with Nightingale's Swim School once qualified (at least the number of current hours)
If I choose to leave Nightingale's Swim School before the year, payment of their contribution will be paid back on a pro-rata basis
